Medicare Medical Audit
determines if the medical records support the level of care delivered to
your patients and meets Medicare's "reasonable and necessary"
standard.
We provide two levels of services, basic and comprehensive, both of them
will provide an objective overview of the status of the rehabilitation
department in your clinic.
Basic
Audit:
This process comprises
of a medical record audit of both current and discharged
charts along with a comparison of documentation to the claims billed. It
includes a verbal report and normally takes 1 day.
Comprehensive
Audit:
This process
normally takes 1½ – 2 onsite days depending on the level of consulting services required.
At a minimum it
comprises of
an entrance and exit conference
with
management and/or
members
of staff; a
preliminary
report
on findings with recommendations presented at the exit conference,
optional services can be included.
Standard:
1. Entrance Conference: This meeting
will augment the information obtained prior to the visit and can be with
management and /or staff.
2.
Medical
Records Audit: The
review and audit includes both current and discharged patients and
compares documentation to the information on the claims billed.
3. Exit conference:
Presentation of a preliminary report with recommendations and can be
with management and/ or staff.
Optional:
4.
Observation of
rehab staff
to compare the
documentation in the medical record with the treatment provided to
ensure staff are maximizing legitimate reimbursement for their services.
